The practice was established in the 1920s and has been serving the community in Chorleywood and its surrounding villages in Hertfordshire and Buckinghamshire ever since. The purpose-built surgery was opened in 1997 and provides comprehensive general medical services including treatment and prevention of illness, health promotion and screening services. We are active in medical and nursing research, training and education. The practice works within the National Health Service and is a member of the West Hertfordshire Primary Care Trust who manage the provision of general practice in the area.

NEWS AUTOMATED APPOINTMENT TELEPHONE BOOKING SYSTEM STARTING 12 JULY 2011 The surgery is introducing a new automated way of booking, cancelling or checking your doctor's appointments. You telephone the surgery as normal, and choose the option from the menu to use the Automated Booking Service. You will have useful options to ensure that you get an appointment to suit your needs. One of the main benefits is that you can telephone the surgery at any time, even when its closed. You can book appointments, cancel them when necessary or simply use it to check the time of your appointment.
EXTENDED HOURS With effect from June 2011 the Practice will offer extended Opening Hours during the week between 6.30pm to 7.00pm and one Saturday a month between 9.00am to 11.30am. The Extended Opening Hours are for ROUTINE appointments bookable in advance. The assessment and treatment of URGENT CASES will continue with Out of Hours service on Tel No. 03000 333 333
Practice involved in three major research projects Chorleywood Health Centre has been selected to participate in three major research projects that will investigate how technology can be used to better manage patients with chronic diseases. The Hydra project investigates the exciting concept of using the forthcoming smart electricity meters to act as a method to allow patients to make simple measurements of blood pressure or weight and send them automatically so that they can be monitored by practice staff. The Reaction project is investigating advanced techniques to monitor and manage patients with diabetes. The inCasa project is investigating ways to assist frail elderly patients at home.
Hydra project goes live Chorleywood Health Centre has placed its first patient on the Hydra network of smart electricity meters and is now monitoring the weight and blood pressure of the patient in their own home. The system uses Zigbee wireless technology to send readings from the devices from anywhere in the home to the special electricity meter. The meter then sends the reading through to the health centre for staff to review. |
