Confidentiality

The practice adheres to strict confidentiality. Information about patients is exchanged between clinical staff only if relevant to your care.

Young persons under 16 are encouraged to come with a parent or other responsible adult if possible, but will be seen unaccompanied in extenuating circumstances. The confidentiality clause is applicable irrespective of age.

Rights and Responsibilities

Chorleywood Health Centre endeavours to provide healthcare to the resident community irrespective of creed, religion, gender or disability. In return we ask users to show reciprocal respect and consideration.

Patients or their relatives or advocates who are verbally or physically abusive to any member of the staff or who disregard policies made for their own safety will be asked to leave the premises and the practice list.

Complaints

The partners and staff hope that you are satisfied with the care provided. If, however, you feel that this is not the case the following guidelines will assist you.

If you are concerned about a practice-related matter you should contact Mrs Diane Back or Mrs Christine Brown. It may be possible to resolve the matter by telephone or with a personal meeting. You are welcome to bring a relative or friend.

If such a meeting is inappropriate, or takes place without resolving the complaint, then you should write to the Complaints Manager at the local Health Authority. As this practice serves two Health Authorities your address will determine the contact:
