Charges to Patients

There are services that are dispensed by the practice which are not reimbursed by the NHS and for which there is a charge. The reception desk can advise on cost. Examples are:

• Driving Medicals
• Provident Association claim forms
• Fitness to travel statements
• Holiday cancellation statements
• Private sick notes (less than 5 days off)
• Passport countersigns
