Community Nursing Services

Patients who are house bound are able to access the community nursing service.

The nurses are able to assess the level of nursing need and provide appropriate equipment, advice and support.

The team is committed to looking after people in their own homes, and to reduce the need for hospital admission if possible.

The team is also aware of the contribution made by family carers and are available to support carers.

The team are able to sign post to other services if appropriate, and are able to refer to other community, social and therapy services.

The nursing service is available 365 days per year, has access to evening and all night nursing services if required, to assist with out of hours care.

The nursing team can link to specialist nurses, who are experts in continence care for example, Macmillan nurses who help the Nursing team to care for patients with life limiting illnesses.

Because of the excellent facilities at Chorleywood Health Centre, nursing can be undertaken in the clinic, but the same care can be carried out in the home if the patient is unable to attend the surgery for any reason.

The nursing team are skilled in tissue viability, leg ulcer management and care, drug administration, palliative care monitoring and management of long term conditions.
